School Forms Completion of these forms is required before your child begins attending The Methodist Preschool. Enrollment Form Please fill the enrollment form out completely including every line, with an answer or a slash to acknowledge that you have completed that line. Please send your enrollment form and your $250 deposit/registration check* to: Kile Kenny 20 South Franklin Street Chagrin Falls, OH 44022 *Please make your check payable to United Methodist Church of Chagrin Falls. Medical Form The medical form must be completed and signed by your child's physician and will be kept on file at the preschool as required by our licensing policy. Physical exams are good for one year. Most family insurances allow one "well" visit per year so please submit the medical form to your child's pediatrician and have them sign and date the form when your child was last examined by their doctor. The preschool will supply a new medical form one month before your child's next birthday. Your child's completed medical form is due the first week of preschool along with your first tuition payment.
